Chronic Tension Type Headache

Classifying Requirements

The International Headache Society (I.H.S.) classification system for chronic tension-type headache requires the following:

A. At least 15 headache days per month for 6 months fulfilling the criteria in B, C, and D
B. Lasting hours to days, or unremitting
C. At least two of the following:

  • Pressing/tightly (non-pulsating) quality
  • Patient able to maintain mild or moderate activity (i.e. not severe – incapacitating)
  • Bilateral location
  • Headache not aggravated by walking stairs, similar routine or physical activity

C. Both of the following:

  • Neither moderate or severe nausea or vomiting
  • Only one of following:
    • Photophobia (light is uncomfortable)
    • Phonophobia (noise is uncomfortable)
    • Nausea
